Mân Anhwylderau
Cyn i chi drefnu apwyntiad i weld meddyg, ystyriwch a allwch chi helpu eich hun.
Os oes gennych beswch, annwyd, cur pen neu fân anhwylder arall ceisiwch drin
eich hun gartref yn gyntaf. Dylech ddechrau teimlo'n well ymhen rhyw 1 i 2
wythnos heb fod angen gweld meddyg teulu. Mae ystod eang o feddyginiaethau
dros y cownter ar gael i'w prynu.

Mordwyo Gofal
Mae Practisau Meddygol ledled Cymru yn gweithredu system o’r enw ‘Care
Navigation’ a hyn er mwyn gwneud y defnydd mwyaf effeithlon o adnoddau a
sicrhau bod cleifion yn cael eu cyfeirio at y gweithiwr gofal iechyd proffesiynol
mwyaf priodol i’w helpu.
Pan fyddwch yn cysylltu â ni i drefnu apwyntiad, bydd ein derbynyddion yn gofyn i
chi amlinellu'n fyr y rheswm dros eich galwad. Os yw'n ymholiad anfeddygol
mae'n ddigon posibl y bydd y derbynnydd yn gallu helpu neu yn hytrach na
meddyg teulu, efallai y cewch eich cyfeirio at Nyrs y Practis, ffisiotherapydd,
fferyllfa neu wasanaeth allanol arall.
Mae gweithio yn y modd hwn yn galluogi adnoddau i gael eu defnyddio yn y
ffordd fwyaf effeithlon ac mae'n rhyddhau'r meddygon teulu i ofalu am y cleifion
mwyaf anghenus a'r rhai â chyflyrau cymhleth neu ddifrifol y gellir eu trin gan y
Meddyg Teulu yn unig.
Pwy sydd ar gael yn y practis?
Nyrs Practis (PN)
Mae Nyrs y Practis yn weithiwr iechyd proffesiynol cofrestredig sy'n gweithio ar y
cyd â'r meddygon teulu i reoli gofal ac iechyd ein cleifion. Mae yn asesu, sgrinio,
trin ac addysgu cleifion ac yn goruchwylio gwaith y Cynorthwywyr Gofal Iechyd.
Cynorthwyydd Gofal Iechyd (HCA)
Mae'r Cynorthwy-ydd Gofal Iechyd yn cefnogi Nyrs y Practis a'r tîm clinigol cyfan,
gan gyflawni dyletswyddau cyffredinol a gweithdrefnau penodol.
Ffisiotherapydd
Mae ffisiotherapi cyhyrysgerbydol yn ymwneud â ffisiotherapi'r system
gyhyrysgerbydol. Mae hyn yn cynnwys cyhyrau, esgyrn, cymalau, nerfau,
gewynnau, cartilag a disgiau asgwrn cefn.
Os teimlwch eich bod yn dioddef o gŵyn cyhyrysgerbydol a allai elwa o
ffisiotherapi gallwch drefnu apwyntiad trwy ffonio'r dderbynfa. Nid oes angen i
chi weld y meddyg teulu yn gyntaf. Cynhelir clinig bob ddydd Llun.
Bydwraig Gymunedol
Mae'r Fydwraig yn rhoi cyngor, gofal a chymorth i fenywod a'u teuluoedd tra'n
feichiog, drwy gydol y cyfnod esgor ac yn ystod y cyfnod ar ôl genedigaeth babi.

Ymwelydd Iechyd
Mae’r Ymwelydd Iechyd yn darparu cymorth i blant 0 i 5 oed a’u teuluoedd, gyda’r
nod o roi’r dechrau gorau mewn bywyd iddynt

remember that seeing the GP isn’t always the best option.

Minor Ailments
Before you book an appointment to see a doctor please consider if you can help
yourself. If you have a cough, cold, headache or other minor ailment try treating
yourself at home first. You should begin to feel better in about 1 to 2 weeks
without the need to see a GP. A whole range of over the counter medicines are
available to buy.

find out the next steps to take.
Care Navigation
Medical Practices across Wales operate a system called ‘Care Navigation’ and this
in order to make the most efficient use of resources and to ensure that patients
are directed to the most appropriate health care professional to help them.
When you contact us to arrange an appointment, our receptionists will ask you to
briefly outline the reason for your call. If it’s a non-medical query it’s quite
possible that the receptionist will be able to help or rather than a GP, you may be
directed to the Practice Nurse, physiotherapist, pharmacy or another external
service.
Working in this way enables resources to be used in the most efficient way and it
releases the GPs to care for the patients most in need and those with complex or
serious conditions which can be treated by the GP only.
Who’s available at the practice?
Practice Nurse (PN)
The Practice Nurse is a registered health professional who works collaboratively
with the GPs to manage the care and health of our patients. The PN assess,
screen, treats and educates patients and supervises the work of the Healthcare
Assistants.
Healthcare Assistant (HCA)
The Healthcare Assistant supports the Practice Nurse and the clinical team as a
whole, undertaking general duties and specific procedures.
Physiotherapist
Musculoskeletal physiotherapy relates to physiotherapy of the musculoskeletal
system. This involves muscles, bones, joints, nerves, ligaments, cartilage and
spinal discs.
If you feel that you are suffering from a musculoskeletal complaint that could
benefit from physiotherapy you can arrange an appointment by phoning
reception. You do not need to see the GP first. A clinic is held every Monday.
Community Midwife
The Midwife provides advice, care and support to women and their families while
pregnant, throughout labour and during the period after a baby’s birth.
You can be referred by a GP or contact the Midwife directly. To contact, please call

Health Visitor
The Health Visitor provides support for children aged 0 to 5 years and their
families, with the aim of giving them the best start in life.
The health visitor is based at Bala Health Centre. The appointments are arranged
