Cludiant ar gyfer Apwyntiadau Ysbyty : Gwasanaethau Cludo
Cleifion Di-argyfwng (NEPTS)
Mae’r Gwasanaeth Cludo Cleifion Di-argyfwng ar gyfer cleifion ledled Cymru nad
ydynt yn gallu gwneud eu ffordd eu hunain i ac o’u hapwyntiadau ysbyty am
resymau meddygol. Mae yna broses gymhwysedd y mae'n ofynnol i bob claf fynd
drwyddi er mwyn sicrhau y gellir cynnig y gwasanaeth iddynt. ( Ymddiriedolaeth
GIG Gwasanaeth Ambiwlans Cymru ) .

Os nad ydych yn gymwys am gludiant, byddwch yn cael cynnig rhifau cyswllt eraill
ar gyfer sefydliadau a allai eich cynorthwyo.
Transport for Hospital Appointments : Non Emergency
Patient Transport Services (NEPTS)
The Non-Emergency Patient Transport Service is for patients across Wales who
are unable for medical reasons, to make their own way to and from their hospital
appointments. There is an eligibility process that all patients are required to go
through to ensure the service can be offered to them. (Welsh Ambulance Service
NHS Trust)
Visit the NEPTS web page or call 0300 123 2303, to check your eligibility.
If you are not eligible for transport, you will be offered alternative contact
numbers for organisations that may be able to assist you.

chi'n gofalu amdanoch chi'ch hun ac yn cymryd yr holl ragofalon angenrheidiol i
amddiffyn eich iach tra byddwch chi i ffwrdd. Gall ein Nyrs Practis eich cynghori
ynghylch unrhyw frechiadau y gallai fod eu hangen arnoch.
Mae'n bwysig eich bod yn cysylltu â ni ymhell cyn eich dyddiad teithio arfaethedig.
Mae angen rhoi rhai brechlynnau ymhell ymlaen llaw i ganiatáu i'ch corff
ddatblygu imiwnedd ac mae brechlynnau eraill yn cynnwys nifer o ddosau wedi'u
gwasgaru dros sawl wythnos neu fisoedd. Mae rhai brechlynnau am ddim ar y
GIG, rydym yn codi ffi am eraill.
Yn gyntaf, cwblhewch ein holiadur iechyd teithio. Gyda'r manylion o'r holiadur
bydd y nyrs yn ymchwilio i ba ragofalon sydd eu hangen ac yna'n cysylltu â chi i
drafod a rhoi gwybod i chi am y gost. Caniatewch 5 diwrnod gwaith ar gyfer y
broses hon. Os ydych chi'n hapus i barhau, bydd apwyntiad yn cael ei wneud a
bydd y brechiadau'n cael eu harchebu. Bydd angen talu ymlaen llaw.
Nid ydym yn ganolfan dwymyn felen felly ni allwn frechu yn erbyn y dwymyn
felen.

If you are planning to travel or work abroad, it is very important that you look
after yourself and take all the necessary precautions to protect your health while
you’re away. Our Practice Nurse can advise you about any vaccinations that you
may need.
It’s important that you contact us well in advance of your planned travel date.
Some vaccines need to be given well in advance to allow your body to develop
immunity and other vaccines involve a number of doses spread over several
weeks or months. Some vaccines are free on the NHS, we charge a fee for others.
Firstly, please complete our travel health questionnaire. With the details from the
questionnaire the nurse will research what precautions are needed and then get
in touch with you to discuss and advise you of the cost. Please allow 5 working
days for this process. If you’re happy to proceed an appointment will be made
and the vaccinations ordered. Payment will be required up front.
We are not a yellow fever center so cannot vaccinate against yellow fever.
