yn cynnig gwasanaethau galw i mewn.

Os nad ydych yn siŵr pa fath o apwyntiad a gyda phwy, sydd fwyaf priodol,
byddem yn fwy na pharod i roi cyngor.
Canslo apwyntiad
Mae canslo eich apwyntiad ymlaen llaw nid yn unig yn caniatáu i ni gynnig amser i
glaf arall sydd angen gofal, ond hefyd yn eich helpu i osgoi cael eich marcio fel
‘Heb Fynychu’ (DNA) o dan ein polisi practis. Caiff apwyntiadau a gollwyd eu
monitro a chymerir camau gweithredu os daw patrwm i'r amlwg a all arwain at
eich tynnu oddi ar ein rhestr cleifion.
Sut i ganslo eich apwyntiad

Mordwyo Gofal
Mae GP Care Navigators yn aelodau hyfforddedig o'r tîm sy'n helpu cleifion i ddod
o hyd i'w ffordd drwy'r system gofal iechyd a chael mynediad at y gwasanaethau
a'r cymorth cywir ar gyfer eu hanghenion iechyd.
Gofynnir i'n llyw-wyr gofal ofyn cwestiynau ynghylch pam yr ydych yn ffonio fel y
gellir eich cyfeirio at y person cywir i helpu. Gall hyn olygu eich cyfeirio at
wasanaethau eraill y tu mewn neu'r tu allan i'r practis. Mae hyn yn gwneud y
defnydd mwyaf effeithlon o'n gwasanaethau ac yn sicrhau eich bod yn cael eich
gweld gan y gwasanaeth mwyaf priodol a chyn gynted â phosibl.
Nid yw gweld y meddyg teulu bob amser yn angenrheidiol oherwydd gellir rheoli
llawer o faterion iechyd yn effeithiol heb ymweliad meddyg. Ar gyfer mân
anhwylderau fel annwyd, cur pen, neu broblemau stumog, mae strategaethau
hunanofal a thriniaethau dros y cownter yn aml yn ddigonol. Yn ogystal, gall
gweithwyr gofal iechyd proffesiynol eraill, nyrsys o'r fath, roi arweiniad a
chymorth ar gyfer pryderon iechyd cyffredin. Gall y dull hwn arbed amser a
sicrhau bod apwyntiadau meddygon teulu yn cael eu cadw ar gyfer materion
iechyd mwy cymhleth neu ddifrifol sydd angen sylw meddygol.
Apwyntiadau Meddygon Teulu
Mae apwyntiadau meddyg teulu yn 10 munud o hyd ac maent i un person
drafod un broblem feddygol.
Gallwch ddewis o apwyntiad wyneb yn wyneb neu ymgynghoriad dros y ffôn ar
adeg archebu. Os oes gennych faterion cymhleth neu luosog i'w trafod,
gofynnwch am apwyntiad dwbl. Efallai y byddwch yn cael dolen fel y gallwch
anfon llun o'ch cwyn at y Meddyg Teulu neu yn yr un modd efallai y byddwch yn
cael gwybodaeth am eich cyflwr trwy e-bost neu ffôn symudol ar ôl i chi siarad â'r
meddyg teulu.
Apwyntiadau arferol
Mae apwyntiadau arferol ar gyfer problemau nad ydynt yn rhai brys neu
apwyntiadau dilynol os oes gennych gyflwr parhaus. Mae'r apwyntiadau hyn ar
gael i'w trefnu o leiaf chwe wythnos ymlaen llaw.
Apwyntiadau ar y dydd
Mae gennym nifer penodol o apwyntiadau ar y dydd ar gael bob dydd. Mae'r
rhain ar gyfer problemau meddygol sydd angen cyngor neu driniaeth yr un
diwrnod.
Apwyntiadau brys
Os ydych chi'n sâl iawn ac angen cael eich gweld yn fuan/ar frys, cysylltwch â'r
practis dros y ffôn cyn gynted â phosibl ac esboniwch eich cyflwr i'r derbynnydd.
Byddwch yn cael cynnig ymgynghoriad yr un diwrnod lle bo'n berthnasol (a allai
fod dros y ffôn).
Plant dan 16 oed
Bydd plant dan 16 oed bob amser yn cael ymgynghoriad yr un diwrnod os ydynt
yn ddifrifol wael (efallai y bydd hyn ar y ffôn).
Ymweliad cartref
Gellir trefnu ymweliad cartref os ydych yn rhy sâl i fynychu'r feddygfa. Eglurwch
yn fyr i'r derbynnydd y rheswm dros eich cais. Bydd y meddyg yn cysylltu â chi ac
yn penderfynu a oes angen ymweliad cartref.
Ymholiadau cyffredinol, ceisiadau gweinyddol a diweddariadau atgyfeirio
Ar gyfer ymholiadau cyffredinol, gwaith papur, ffurflenni y mae angen eu llenwi, a
chwestiynau ynghylch cyfeiriadau, mae ein gweinyddwyr yn y sefyllfa orau i'ch
cynorthwyo. Mae ganddyn nhw'r arbenigedd i ddarparu atebion amserol a
sicrhau bod eich ceisiadau'n cael eu trin yn effeithlon. Felly, peidiwch â gwneud
apwyntiad gyda’r meddyg teulu ar gyfer y materion hyn, oherwydd gallai oedi eich
ymateb a chymryd amser i ffwrdd oddi wrth gleifion sydd angen sylw meddygol.
Apwyntiadau Nyrs Practis / Cynorthwyydd Gofal Iechyd
Mae'n bwysig dweud wrth y derbynnydd beth yw'r rheswm dros eich apwyntiad
wrth archebu gan fod amodau ac adolygiadau yn gofyn am hyd apwyntiad
gwahanol a chlinigwyr.
Mae nyrs practis yn darparu amrywiaeth o wasanaethau gofal iechyd i gefnogi
iechyd a lles cleifion, gan gynnwys brechiadau, sgrinio iechyd, rheoli cyflyrau
hirdymor, a chefnogi cleifion gyda chyngor ar ffordd o fyw a mân driniaethau.
Maent yn gyfrifol am roi brechiadau ac imiwneiddiadau plentyndod, cynnal
sgrinio iechyd, gan gynnwys gwiriadau pwysedd gwaed ac asesiadau diabetes. Yn
ogystal â'r tasgau hyn, mae nyrsys practis yn rheoli cyflyrau hirdymor fel asthma,
diabetes, a gorbwysedd trwy fonitro iechyd cleifion, gan eu haddysgu am eu
cyflyrau. Maent hefyd yn darparu cymorth ar gyfer mân driniaethau, megis gofal
clwyfau, tynnu pwyth, a chwistrellu clustiau.
At hynny, mae nyrsys practis yn cynnig cyngor gwerthfawr ar ffordd o fyw, gan
helpu cleifion i roi'r gorau i ysmygu, rheoli pwysau, a chanllawiau maeth. Mae eu

Ffisiotherapydd a Bydwraig
Gellir gwneud apwyntiad yn uniongyrchol gyda'r ffisiotherapydd a'r fydwraig. Nid
oes angen gweld y meddyg teulu yn gyntaf.
Pobl Ifanc: Gweld Meddyg Teulu neu Nyrs yn Unig
Fel plentyn, mae'n gyffredin ymweld â'r meddyg teulu gydag oedolyn. Fodd
bynnag, wrth i blant dyfu’n hŷn, efallai y byddai’n well ganddynt fynychu
apwyntiadau ar eu pen eu hunain, sy’n berffaith iawn. Er y gall plant yn dechnegol
fynychu apwyntiadau ar eu pen eu hunain ar unrhyw oedran, yn gyffredinol
ystyrir 13 oed a hŷn fel yr amser mwyaf priodol i ddechrau.
Byddwch yn dawel eich meddwl, bydd y meddyg neu'r nyrs yn cadw popeth a
drafodwyd yn ystod yr apwyntiad yn gyfrinachol, oni bai bod y person ifanc yn
gofyn iddynt rannu'r wybodaeth neu os oes pryderon am eu diogelwch neu
ddiogelwch eraill.

Our surgery operate on an appointment-only system, and we do not offer walk-in
services.

If you’re unsure what type of appointment and with who, is most appropriate,
we’d be more than happy to advise.
Cancelling an appointment
Cancelling your appointment in advance not only allows us to offer the time to
another patient in need of care, but also helps you avoid being marked as a ‘Did
Not Attend’ (DNA) under our practice policy.

Care Navigation
GP Care Navigators are trained members of the team who help patients find their
way through the healthcare system and access the right services and support for
their health needs.
Our care navigators are requested to ask questions about why you are calling so
that you can be directed to the right person to help. This can mean directing you
to other services within or outside the practice. This makes the most efficient use
of our services and ensures that you get seen by the most appropriate service
and as soon as possible.
Seeing the GP isn’t always necessary because many health issues can be
effectively managed without a doctor’s visit. For minor ailments like colds,
headaches, or stomach issues, self-care strategies and over-the-counter
treatments are often sufficient. Additionally, other healthcare professionals, such
nurses, can provide guidance and support for common health concerns. This
approach can save time and ensure that GP appointments are reserved for more
complex or serious health issues that require medical attention.
GP Appointments
GP appointments are 10 minutes in length and are for one person to discuss
one medical problem.
You can choose from a face to face appointment or telephone consultation at
time of booking. If you have a complex or multiple issues to discuss, please ask
for a double appointment. You might be provided with a link so that you can
send a photo of your complaint to the GP or similarly you may be provided with
information about your condition via email or mobile phone once you have
spoken to the GP.
Routine appointments
Routine appointments are for non-urgent problems or follow ups if you have an
ongoing condition. These appointments are available to book at least six weeks in
advance.
On the day appointments
We have a certain number of on the day appointments available daily. These are
for medical problems which require same day advice or treatment.
Emergency appointments
If you’re very unwell and need to be seen soon/urgently, contact the Practice by
phone as soon as possible and explain your condition to the receptionist. You’ll
be offered a same day consultation where relevant (which may be over the
phone).
Children under 16
Children under 16 will always be given a same day consultation if they are acutely
unwell (this may be on the phone).
Home visit
A home visit may be arranged if you are too unwell to attend the surgery. Please
explain briefly to the receptionist the reason for your request. The doctor will
contact you and decide if a home visit is necessary.
General enquiries, administration requests and referral updates
For general enquiries, paperwork, forms that need to be completed, and
questions regarding referrals, our administrators are best equipped to assist you.
They have the expertise to provide timely answers and ensure your requests are
handled efficiently. Therefore, please refrain from making an appointment with
the GP for these matters, as it may delay your response and take time away from
patients needing medical attention.
Practice Nurse / Healthcare Assistant (HCA) appointments
It’s important to tell the receptionist the reason for your appointment when
booking as different conditions and reviews require different appointment
lengths and clinicians.
A practice nurse provides a range of healthcare services to support patient health
and well-being, including vaccinations, health screenings, managing long-term
conditions, and supporting patients with lifestyle advice and minor treatments.
They are responsible for administering vaccinations and childhood
immunisations, conducting health screenings, including blood pressure checks
and diabetes assessments. In addition to these tasks, practice nurses manage
long-term conditions like asthma, diabetes, and hypertension by monitoring
patients’ health, educating them about their conditions. They also provide
support for minor treatments, such as wound care, stitch removal, and ear
syringing.
Furthermore, practice nurses offer valuable lifestyle advice, helping patients with
smoking cessation, weight management, and nutrition guidance. Their expertise
and support are essential in promoting preventive care and improving overall
health outcomes in the community.
Physiotherapist and Midwife
An appointment can be made directly with the physiotherapist and midwife.
There is no need to see the GP first.
Young People: Seeing a GP or Nurse Alone
As a child, it’s common to visit the GP with an adult. However, as children grow
older, they may prefer to attend appointments on their own, which is perfectly
fine. While children can technically attend appointments alone at any age, 13 and
above is generally considered the most appropriate time to start.
Rest assured, the doctor or nurse will keep everything discussed during the
appointment confidential, unless the young person asks them to share the
information or if there are concerns about their safety or the safety of others.
